# Lockout authorization form

A one-page form the customer signs before you open the door: photo ID, proof of ownership and a signed statement. Home, car or business, in English or Spanish.

The form opens with an **example shop**. Put in your own details; the customer fills in the rest by hand.

## The form

Changes the questions and the proof list.

Some states ask for more. California's locksmith law, for example, wants the full ID number, date of birth and the customer's address on home and business lockouts. Check your state's rule.

## Your shop

## How to use a lockout authorization form

A lockout authorization form is the paper that protects you when someone asks you to open a door that turns out not to be theirs. The customer writes who they are, shows a photo ID and proof they belong there, and signs a statement asking you to open it. You keep the signed form with the job.

To use the tool, pick what you're opening (home, car or business), the language and how much of the ID to record. Add your shop name, phone and license number, then print a stack for the van. The form always fits on one page: if your shop details make it long, it shrinks a little to fit.

### What a lockout authorization form should include

| Section | What it records | Why it matters |
| --- | --- | --- |
| Customer | Name, phone and the address opened. The business form adds the business name and their role. | Ties the job to a person you can find later. |
| Vehicle (car form) | Year, make, model, color, plate and VIN. | The VIN on the papers must match the VIN on the dash. |
| Photo ID | ID type, who issued it, and the last 4 digits, or the full number, date of birth and home address. | Shows you checked who they are. |
| Proof checklist | Lease, utility bill, registration, title, business license and more. | Shows they have the right to enter, not just a name. |
| Signed statement | They own it or have the owner's permission, and ask you to open it. | Puts the claim in their words, with their signature. |
| Trip fee notice | What they owe if they can't show ID and proof (optional). | Stops arguments when you have to walk away. |
| Technician | Your tech's name and signature. | Shows who did the job and checked the papers. |

### What counts as proof of ownership

- **Home or apartment:** a lease, a utility bill or mail addressed to them at that address, a mortgage statement or property tax bill, an ID that shows the address, or a letter from the owner or property manager.
- **Car or truck:** the registration, the title, an insurance card that lists that vehicle, a rental agreement, or lease or loan papers. Check the VIN on the papers against the VIN at the base of the windshield.
- **Business:** a business license in that name, the lease for the premises, a utility bill in the business name, a company ID badge, or a call or letter from the owner or property manager confirming access.

A photo ID alone proves who someone is, not that they belong inside. Ask for both. Our field guide on [proof of ownership on a lockout](https://lockcommand.com/blog/proof-of-ownership-lockout) covers the hard cases, like a new tenant with no mail yet or a car borrowed from a friend.

### When the ID and papers are locked inside

This happens on most car lockouts. Open the door, then have the customer get their ID and registration while you watch. Check them before you pack up, tick "checked right after opening" and write the time. If they can't produce anything, don't hand over the car or the house. Note it on the form and call the police non-emergency line. The optional trip fee line tells them up front that the call still costs money.

### Worked example: a car lockout, start to finish

1. On the phone, tell the caller you'll need a photo ID and the registration or insurance card, and say the trip fee if they can't show them. A [phone script](https://lockcommand.com/tools/phone-script-builder) keeps this the same on every call.
2. On site, hand over the car form. They fill in their name, phone, where the car is, and the year, make, model, plate and VIN.
3. They sign the statement. You open the car.
4. They pull the registration from the glove box. You check the name against their driver's license and the VIN against the dash, and tick what you saw.
5. You tick "checked right after opening", write 9:42 p.m., and sign the technician line. The form goes in the job folder.

### State rules on lockouts and records

Rules differ a lot. Three examples:

- **Texas** bars a locksmith from opening a structure, vehicle or other property unless the customer shows government-issued ID and gives a signed authorization that they own it or have legal access, unless someone is in immediate danger ( [Occupations Code 1702.2865](https://texas.public.law/statutes/tex._occ._code_section_1702.2865) ). This form's signed statement is that authorization.
- **California** requires a work order for opening a home or business with the customer's name, address, phone, date of birth, driver's license or ID number and signature. Making keys for a car needs the vehicle's registration or identification number too. Each work order shows the locksmith's license number and is kept for two years ( [Business and Professions Code 6980.55](https://california.public.law/codes/ca_bus_and_prof_code_section_6980.55) ). A California shop should pick **Full ID number, date of birth and address** .
- **Illinois** wants the customer's name, address and phone and the locksmith's name and license number on the work order, invoice or receipt, with the original kept for two years ( [225 ILCS 447/30-30](https://www.ilga.gov/legislation/ilcs/documents/022504470K30-30.htm) ).

Many other states license locksmiths without setting form rules. See [locksmith license rules by state](https://lockcommand.com/tools/locksmith-license) for your board, and record the job on a [work order](https://lockcommand.com/tools/work-order-and-estimate) as well when your state asks for one.

### Spanish lockout form

Pick Spanish and every line changes, including the proof list and the statement, so a Spanish-speaking customer knows exactly what they are signing. The wording is plain US Spanish ("licencia de conducir", "comprobante de propiedad"). For the phone call before the job, see [Spanish for locksmiths](https://lockcommand.com/tools/spanish-for-locksmiths).

### Keep it legal and keep it safe

**This form isn't legal advice.** It's a practical template. Have a local attorney review your form before you rely on it. Signed forms hold names, ID details and addresses, so store them somewhere locked and shred them when your retention period ends.

## Questions

### Does a locksmith need ID to open a car or a house?

A careful one always asks. Some states make it the law: Texas, for example, bars a locksmith from opening a building or vehicle until the customer shows government ID and signs an authorization saying they own it or have legal access. Elsewhere it is still the best protection you have if the job turns out to be a break-in.

### What counts as proof of ownership for a lockout?

For a home: a lease, a utility bill or mail at that address, a mortgage statement, or an ID showing the address. For a car: the registration, title or an insurance card for that vehicle, with the VIN matching the dash. For a business: a business license, the lease, or a call or letter from the owner or property manager.

### What if the customer's ID and proof are locked inside?

Many locksmiths open first, then check ID and proof before they leave, with the customer in sight. The form has a box for that: tick it and write the time you checked. If they can't show anything once it's open, close up and call the police non-emergency line.

### Is this form legally required?

It depends on your state. California and Illinois set what goes on a locksmith's work order and keep it for two years, Texas requires a signed authorization, and many states say nothing. This form is a practical template, not legal advice. Have a local attorney review the form you use.

### Why only the last 4 digits of the ID?

A full ID number on a paper form is an identity theft risk if the form is lost. The ID type plus the last 4 digits show you checked. If your state asks for more, pick Full ID number, date of birth and address.

### How long should I keep signed lockout forms?

Follow your state's rule first: California and Illinois say two years. If your state has no rule, two years is a sensible floor. Keep them locked away, because they hold personal details.

### Is the Spanish form a direct translation?

It says the same things in plain Spanish written for customers in the US, and the statement and checklist match the English form line for line. Nothing you type is sent or saved; print it or save it as a PDF.
